Child's Registration Form

There are two options to complete this form:

1- Download the pdf form, print, manually complete and bring to event registration.

or

2 – Complete the online form, send to us, print a copy and bring to event registration.

By completing the online form, you allow us to prepare for the number of children in advance, and have your child on a list.

You still need to bring your child and the completed forms with you when you register.

Tips and Reminders

Tips and Reminders:

– 5 Trout Limit per registered child
– Free bait & tackle while supplies last
– You can bring your own bait & tackle but no Treble Hooks Allowed!
– Children and Parents should wear water-friendly shoes and shorts.
– Bring towels, snacks for the family, a cooler for fish and portable chairs.
– For pre-registered children ages 3 – 11
