One entry form per child. Submit, then Print and bring to Registration Day. 

Children must be PRESENT to register.

Registration is ONLY available Friday before Trout Derby Event, or morning of Event, or until we have reached our participant limit (2,500).
THERE IS NO REGISTRATION UNTIL Friday, August 4.
Bring this Registration Form and the Liability Release Form with you to register.  Only 2,500 entries MAX (for poles and t-shirts) so register early!

Pre-registration does guarantee your child will receive free items if you are in line by 8 AM on Saturday when fishing starts. Otherwise, first come, first served.

Day of Event, your child will receive:
FREE BREAKFAST AND LUNCH for all registered children.
FREE Poles, Bait and everything your child need to fish.
FREE T-shirt and Trout Derby swag from sponsors.

There will be a 5 FISH LIMIT per registered child.
Bait and hooks will be provided while supplies last. You can bring your own-single hooks only. 

Submit this form, then print a copy to bring on Registration Day.
